We will send you a “Request Form for Disclosure, etc. of Personal Information (for Personal Application),” so please send it to us by mail with a copy of a document that verifies your identity.
After confirming your identity, we will respond to your request for disclosure, etc.In the case of a proxy, please attach a letter of attorney and a copy of the proxy’s driver’s license or other document that verifies the proxy’s identity. (If your legal domicile is indicated, please fill in that part of the document even if it is inconvenient.)
②Procedure in person at our office
Please bring documents that verify your identity. After confirming your identity, please fill in the “Request Form for Disclosure of Personal Information (for Personal Application)” and we will respond to your request for disclosure, etc.
In the case of a representative, please bring a letter of attorney and a document that verifies the representative’s identity, such as a driver’s license.
※We will destroy the “Request Form for Disclosure of Personal Information (for individual application)” after the procedure is completed by mail or in person.
4.How to notify

5.Reason for nondisclosure
If any of the following items ① through ⑦ applies, we will not respond to the request for disclosure. If we are unable to comply, we will notify you to that effect and the reason in writing.
①If the name and address on the “Request Form for Disclosure of Personal Information (for individual application)” do not match the name and address on the documents for identification
②If the name and address of the representative listed in the “Request Form for Disclosure of Personal Information (for the applicant)” do not match the name and address listed in the documents for identity verification
③Cases in which the information in the Request for Disclosure, etc. does not fall under the category of Personal Information Subject to Disclosure
④When there is a risk of harm to the life, body, or property of the individual or a third party if the existence or nonexistence of the personal information requested for disclosure, etc., is revealed.
⑤Where there is a risk that the disclosure of the existence or nonexistence of the personal information for which a request for disclosure, etc. has been made may encourage or induce illegal or unjust acts.
⑥Where there is a risk that national security may be impaired or the relationship of trust with other countries or international organizations may be damaged or negotiations may be disadvantageous if the existence of the personal information for which a request for disclosure, etc. has been made becomes known.
⑦By revealing the existence or nonexistence of the personal information for which a request for disclosure, have been made,When there is a risk that the prevention, suppression, or investigation of crimes or the maintenance of other public safety and order may be hindered by the disclosure of the existence or nonexistence of the personal information for which disclosure is requested.
